The Multi-Family Service Program (MSP) is designed to assist residents of Public Housing to become or remain self-sufficient and more knowledgeable of the resources available in the Yuma community. The program links seniors and persons with disabilities to essential community supportive services.

There are many common needs for older and disabled residents. The most common is to remain in their home. With the help of the MSP, many residents are granted that wish and can continue living in reasonably independent situations.

 

Who The Program Serves

The work of service coordinators aids three groups of residents in HACY public housing communities:

  • Low-income seniors who need support to age in place in their own homes
  • Individuals with disabilities who need assistance maintaining independence
  • Low-income families working toward self-sufficiency and economic independence

Service Coordinator

The service coordinator position provides information and referrals to residents who need support services to maintain self-sufficiency. The coordinator identifies, locates, and connects residents to services that fit their individual needs.

Services Offered

The MSP offers an initial needs assessment to determine unmet needs, then provides referrals and coordination across a broad range of community services.

 

Medical and Mental Health Referrals

Referrals to medical providers and mental health services for residents who need professional care.

Meal and Nutritional Services

Connections to meal programs and nutritional assistance for residents and their children.

 

Emergency Response Services

Access to emergency response resources so residents can get help quickly when they need it most.

Educational Workshops

Workshops covering topics that help residents build life skills, improve financial management, and access community resources.

 

Employment Information

Employment resources and referrals for residents interested in entering or returning to the workforce.

Transportation Services

Transportation assistance provided for qualified participants who need help getting to appointments and services.

 

How The Program Helps Each Group

 

Seniors and Disabled Adults

Service coordinators assist seniors and disabled adults in accessing supportive services that allow them to maintain their independence and remain in their homes, and help them avoid premature admission to more costly institutionalized care.

Low-Income Families

Service coordinators link adults to programs that improve literacy, job skills, education, and money management. They also connect children to after-school programs and nutritional assistance to support their development and help families move out of poverty.

Property and Community

Service coordinators help resolve lease violations before they become a bigger problem and foster a stronger sense of community at affordable housing complexes. This helps decrease crime and increase safety across HACY's properties.
